Veterans deserve to leave mental health care with clear instructions, informed medication choices, and a VA system that proves those safeguards work.
A veteran who leaves inpatient mental health care needs to understand what comes next. Which medicine treats which condition? Where is the next appointment? Who can answer a question after discharge?
An October 2026 VA inspector general review identified recurring gaps in understandable discharge instructions and documentation of medication risk-benefit discussions.1 Veterans deserve answers they can use.
Protect Informed Choices
VA announced additional medication-consent requirements for 2027, including standardized education and documented discussions about psychotropic medications.2 This progress needs clear implementation deadlines and independent checks.
A separate inspector general investigation at the VA Southern Nevada Healthcare System found that staff failed to obtain informed consent before a patient entered a locked mental health unit.3 An understandable explanation matters throughout care, including the transition home.
Require One National Standard
Ask the Acting Under Secretary for Health to issue a national directive that establishes minimum requirements for every inpatient mental health discharge.
Each veteran must receive a plain-language plan with appointment dates, understandable locations, medication names and purposes, clear directions, and contact information for questions or urgent concerns. Staff must review the plan with the veteran and confirm understanding through a respectful conversation.
The directive must also require timely documentation of medication-consent discussions, with appropriate provisions for emergencies and patients who lack decision-making capacity. Staff need practical templates, training, and enough time to explain care.
Make Compliance Public
Require facility audits and public reports that show compliance rates, corrective actions, responsible leaders, and deadlines. Protect patient privacy. Measure whether veterans receive understandable explanations rather than count completed forms alone.
Sign the petition to call for clear VA mental health discharge plans, documented medication consent, and public accountability at every facility.
